What 2D resolution are you using out of curiosity?
What version of Windows?
Do you have the file "Last_Resort_v0.7.zip" in your mods folder? This file contains the m3nu.dat file.
Please check that in Swat.ini there is the following line:

Actually I fixed it by running Swat.exe and then running Last Resort. No more issues.

Can you explain what happened before this error occurred?
* Fresh Swat 3 install?
* Old install but never installed Last Resort with it before
* Old install and just upgraded Last Resort

Hmmm strange that you got that error since I have tested that upgrade. I suspect for some reason that the mod wasn't activated in your mod folder for some reason.
I had to move some of Last Resort files into the mod folder like a traditional mod because we discovered that the Swat 3 developers used some textures in multiple maps without renaming them. I was having texture priority problems and this was only way to fix.
Just so you know that since Windows 8 & 10 use shims to make Swat 3 run better it was necessary to make the Last Resort mod rename our hacked executable to "swat.exe" depending on what 2D resolution you chose. What the Last Resort Launcher does now is rename the original "swat.exe" to "swat_original.exe" as a backup and if you choose 1920x1080 as your 2D resolution it renames our hacked executable "swat_1920x1080.exe" to "swat.exe" and then starts the game.
